By enrolling in the 8-week program offered by Functional Self Care™ (the “Company”), you agree to the terms outlined in this Agreement. You understand that you’ll be receiving specialized health education services (the “Services”) and agree to follow all guidance provided by the Certified Health Education Specialist.

INFORMATION AND EDUCATION ONLY

The information shared by Functional Self Care™ is intended for educational and informational purposes only. It is general in nature and does not substitute for medical advice, diagnosis, or treatment. We are not practicing medicine, and there is no provider-patient relationship created by using our services. Even if our staff hold licenses or credentials in health-related fields, our role is solely to provide information and education—not clinical services. Our services should not be used to diagnose or treat any health condition. The content shared is not exhaustive and is only one of many possible resources.

NO MEDICAL OR PSYCHOLOGICAL SERVICES

By using Functional Self Care™, you understand that we do not provide medical, pharmacological, or psychological treatment. We do not diagnose, treat, or claim to cure any physical or mental health conditions. Our services are not a substitute for conventional medical or psychological care, and we do not handle emergencies. You are responsible for making your own healthcare decisions in consultation with licensed medical professionals. If anything in our services causes distress or concern, please seek immediate care from a qualified provider. Always consult a licensed healthcare provider before starting or changing any treatments, medications, or routines.

NOT REPLACING CURRENT MEDICAL CARE

Functional Self Care™ does not replace the care provided by your current doctors, including your primary care physician or specialists. We do not treat hospitalized patients and do not provide emergency services. You should continue care with your regular provider and seek emergency care when needed. Annual physical exams and appropriate follow-ups with your healthcare provider are recommended.

DIETARY SUPPLEMENTS

If Functional Self Care™ discusses dietary supplements, know that these are not intended to treat or cure disease. While supplements may support general health, claims about their effectiveness are limited by law. Some supplements may cause side effects, especially when taken with medications. Always talk to your healthcare providers about any supplements or medications you are taking.

CONFIDENTIALITY

You agree not to share or copy any information learned through Functional Self Care™ for personal gain, to create your own program, or to teach others. Doing so could harm both our business and those you share it with, and may result in legal action.

INTELLECTUAL PROPERTY

Any materials, tools, or content shared by Functional Self Care™ are protected by copyright and/or trademark laws. You agree not to reproduce, share, or distribute them in any way, except for your personal educational use.

PRIVACY POLICY

As Functional Self Care™ is not a medical provider, laws like HIPAA may not apply. However, we take your privacy seriously and will protect your personal information. We only share information as needed to deliver services or comply with the law.

RECORDING

By participating in any live event (such as coaching sessions or webinars), you agree that Functional Self Care™ may record the session. We own all rights to these recordings. You may not record or share any sessions or materials without our written permission.

NO CLAIMS OR GUARANTEES

Functional Self Care™ does not promise or guarantee results. While many clients benefit from our services, results vary by individual. We cannot control unexpected events or challenges. We make no claims that any condition will be cured or improved.

REFERRALS

Functional Self Care™ may refer you to other professionals or companies for additional services. We are not responsible for their actions, and they are not employees or affiliates of our company. You are responsible for any agreements you make with them.

RIGHT TO DISCONTINUE SERVICES

Functional Self Care™reserves the right to stop providing services at any time for any reason. If we end services, you will be refunded for any unused portion.

CANCELLATION AND REFUND POLICY

You may cancel your program by notifying us in writing. Functional Self Care™ does not provide refunds for services rendered. Refunds from third-party companies (if applicable) are at their discretion. We may terminate services for nonpayment, abuse, repeated no-shows, or dishonesty.

ASSUMPTION OF RISK & LIABILITY WAIVER

By using services from Functional Self Care™, you accept all associated risks. You agree to release and hold us harmless from any claims related to your use of our services or referrals. You agree not to take legal action against us if you're dissatisfied with the results.

LIMITATION OF LIABILITY

Functional Self Care™ is not responsible for any damages, including personal injury, financial loss, or indirect effects of using our services. Our liability is limited to the amount you paid us, and any legal claims must be brought within one year. You waive your right to join any class action lawsuit against us.
